Patient confidentiality is of paramount importance in the relationship between patients and healthcare professionals.

We cannot disclose personal information to a third party without the patient's explicit consent unless it is of overall benefit to a patient lacking the capacity to consent, required by law, ordered by a court, or justifiable in the public interest.

There may be times when you might want a relative, friend or carer to discuss confidential information with the practice about you, when you are not able to discuss these issues yourself. In these circumstances, we would only be able to have these discussions with a completed permission form.

You can request whether you want just Test results, Prescriptions or Medical conditions only to be discussed.

Please note that it your responsibility to inform the practice if your wishes change.
