Long Term Conditions

We are changing the way we invite patients for their annual review of their Long Term Conditions (such as diabetes, asthma, COPD, hypertension, etc). We are now trying to line up the annual review with their birth month. This might mean that you are called for annual review earlier/later then before. Please bear with us as we work through the new system as it will take a while to get everyone to get their annual reviews lined up with their birth month.
