
New Patient Registration
If you live within our practice area you are welcome to register with us and our reception staff will be happy to guide you through the procedure. Eligibility can be quickly confirmed from your address so please provide proof by way of a recent utility bill. We also require one form of photo ID
If you don't have a medical card you will need to complete a registration form and a health questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor. Both forms can be downloaded from the above "Registering Online" tab.
If you are on any regular medications we will ask you to make an appointment to see the Healthcare Assistant for a patient health check. This is to ensure your medication list is up to date and also to arrange any blood tests or blood pressure checks as necessary.
Once you have been accepted as a patient your medical records will be transferred to us. You do not need to inform your existing practice that you are moving, although it may be courteous to do so. It can take a few weeks for your records to be sent to us.
